Use the QCCC/NDT Remittance Report
to calculate and remit the amount due to the QCCC Trust Fund and NDT Industry Health Benefit and Pension Plans.
If you wish to obtain an online version of the Remittance
Report that will allow you to fill in Member and remittance
information and submit the form electronically, please contact
the Plan Administrator. |

Information Needed to Complete the Form
In addition to the employer information, the following information is needed to complete this form:
- Employee information,
- Local Union affiliation,
- Pension Hours,
- Regular dues, working dues, admin dues and training fund amounts, and
- Calculations to determine the total amount remittable by the employer.
